At-Home Services
Individualized services designed to build skills and foster independence.
Services:
Residential Habilitation
Designed with the person centered approach in mind, the Habilitation Services Department provides one-on-one staffing supports to children and adults. Services provide the opportunity to learn a variety of skills that foster independence, promote social growth, and support the desired outcomes identified by the individual. In general, the services assist individuals in maximizing their independence in various areas of their life. Although these services are usually provided where a person lives, it can encompass a variety of different activities and can occur anytime throughout the day.
Hourly Respite
Hourly respite services provide relief to individuals who are responsible for the primary care and support of individuals with disabilities. Families utilize this service to ensure the needs of their loved ones are met while tending to other obligations. Many individuals who receive this service often have opportunities to participate in fun recreation and social activities within the community.
Weekend Group
The Weekend Group meets every Saturday at the PATH Building. The individuals of this group are then provided with various recreational opportunities throughout the North Country. Trips are often planned for sporting events, festivals, and concerts to such places as Burlington and Lake Placid.
Family Reimbursement
This service provides financial assistance/reimbursement to developmentally disabled individuals and/or their families that care for them. Specifically, the family reimbursement funds assist families with designing, obtaining, and implementing services to meet their family member's needs. Families are able to utilize these funds to lessen the financial strains they are experiencing.
Transportation:
Transportation to and from programs is provided throughout Clinton County.
Eligibility:
Clinton County residents with a Developmental Disability as defined by OMRDD.
Cost:
None to Medicaid eligible individuals.
