Medicaid Service Coordination
This service is required to access all waiver services available to qualified individuals. In addition to accessing waiver services, Service Coordinators offer a variety of supports to the families they serve.




Services

  • Assistance with securing entitlements and benefits such as SSI, food stamps, Medicaid, Medicare, etc.
  • Offer advocacy supports for Individualized Educational Plans (IEP) in the school settings.
  • Develop and maintain the consumer's Individualized Service Plan (ISP) for waiver services.
  • Provide oversight and monitoring of all services received by the individual.
  • Assistance with accessing programs and funding sources for environmental modifications and assistive technology.
  • Crisis intervention and support.


Requirements:

Individuals and family members are required to meet face to face with their Service Coordinator at least once a month to maintain this service. Minimally, twice a year, a planning meeting must occur with all service providers to review services and make necessary changes to ensure all services continue to meet the needs of the individual being served.


Plan of Care Services
Plan of Care Services allows individuals who have received Medicaid Service Coordination supports, to opt out of the monthly meetings and simply meet twice a year to review services and update the Individualized Service Plan. The supports received through regular service coordination are not available to the recipient of Plan of Care Services.



Eligibility:

To qualify for both Medicaid Service Coordination and Plan of Care Services, an individual must have a documented diagnosis of a developmental disability as defined by the Office of Mental Retardation and Developmental Disabilities (OMRDD). The onset of the disability must occur prior to the age of 22.
